Payment Options

At Custom Eyes Optique & Eyecare, our goal is to promote good eye health and enable you to have the clearest vision possible. We accept most types of vision and medical insurance, cash, Care Credit and most credit cards. We inform our patients about exam and treatment costs at the beginning of your appointment. If you require a breakdown of costs, please let us know.

Insurance

The knowledgeable staff at Custom Eyes Optique & Eyecare is experienced in working with vision and medical insurance providers. We will work directly with you and your insurer to submit claims regarding your vision care. We strive to ensure that you understand your insurance coverage and will answer any questions you may have regarding your benefits. Many current vision plans are designed to cover most of the basic vision-related costs, but not all costs associated with eye care. Most plans often require a co-payment for an office appointment as well as additional costs that are covered by the insured patient.

While Custom Eyes will gladly file insurance claims for patients, it is ultimately the responsibility of the patient to determine from their own insurance company if the examination will be a covered service, and if any copays or deductibles will be required.

Our doctors provide both medical eye care and vision care.  Medical insurance plans usually cover services related to the diagnosis and treatment of eye diseases or eye injuries.  Vision insurance is a wellness benefit designed to provide eye examinations, refractions, and prescription eyewear or contact lenses at a reduced cost. Here are just some of the insurance plans that we accept:

VSP, Eyemed, Eyetopia, NVA, Avesis, and more.

Medical Insurance:

Aetna, BCBS, Cigna, Humana, Medicare, United Healthcare (Medical Only), and more.

We do accept other plans as well, so if you do not see your plan listed here, please give us a call and we would be happy to assist you.

Flexible Spending Accounts

Many employers are offering Flexible Spending Account (FSA) options to employees. Sometimes these are referred to as cafeteria plans and are elective, supplemental insurance savings plans. These plans are designed to let you save money in an account, pre-tax, to pay for additional medical expenses such as eye exams, glasses, contacts, and often laser vision surgery. Check with the benefits administrator at your work to see if you are eligible for this program. FSAs can typically be used in conjunction with any vision insurance plan to offset any out-of-pocket costs on your behalf.

Care Credit

As a service to our patients, we are pleased to offer the CareCredit card, the nation's leading patient payment program. CareCredit lets you begin your procedure immediately—then pay for it over time with low monthly payments that are easy to fit into your monthly budget.
